Accessing Medical Records

You have the right to access the information we hold about you.  We must provide the requested information within one month of receiving the request. If this is not possible we will keep you informed of progress.

Online. The simplest way to access your medical record would be to register for online services. This will enable you to view your record as well as request repeat prescriptions and book appointments.  To register for online services you will need to ask at reception.  They may need to see some form of photographic ID to verify you. Alternatively, if we already have an up to date mobile phone number and/or email address for you, then you can phone and ask us to text or email your login details.

In person. You can request to come to the surgery and view your medical records in person.  You will need to put your request in writing to the surgery. One of our admin team will then contact you to make an appointment to come and view your records.  An admin team member will sit with you to show you how to view your record on our system.  Although they may be able to advise you on technical aspects of the system they will not be able to answer any questions you may have about clinical matters.

Paper copies. You can request a paper copy of some or all of your medical record. Please put your request in writing and let us know exactly what you would like to have, e.g. the full record, records between certain dates, all records relating to a specific condition etc. You will be contacted once the requested information is  ready so that you can come and collect it. You may need to bring a photographic form of ID with you as verification.

Paper copies by post.  If you want copies of some or all of your record, but are unable to collect them from the surgery, we can send them via Royal Mail recorded delivery but you will be charged for the postage. Payment may be required in advance.

Charges. You will not be charged for having copies of records we hold about you unless the request is deemed to be unfounded or excessive. For example, if you make multiple requests for the same information or if you request a full medical record for insurance purposes relating to a single specific incident.  As above, if you require us to post the information to you, you will be charged for the postage costs only.  Payment may be required in advance.

Please note: for security reasons we will not put data onto a portable storage device, e.g. USB memory stick, writeable CD etc. provided by you. If you would like us to provide your information in this format we will charge for the cost of the device only. Payment may be required in advance.
